I am a GP working at the surgery Mervelee is registered at. Please note that my previous clinical interaction with Mervelee consists of 3x 10minute consultations since 2017. Please note that time allocated by the surgery for completion of this letter was 20minutes, so I am note able to be as thorough as I would have liked, and I am also not sure that I am going to be able to add much to the thorough psychiatric report that was completed in April. You ask for specific mental health conditions โ she was diagnosed with anxiety and depression several years ago (first coded in 2018). There was a query of bipolar disorder made by my colleague Dr Evans due to paranoid behaviour, pressure of speech, labile mood (please note formal bipolar disorder diagnoses are not generally made by GPs, this is something our psychiatry colleagues do). A CMHT referral was done but rejected, they asked she see the mental health nurse first but it appeared this never happened, although she does now have an appointment at the end of August with the mental health nurse at the surgery โ if the nurse feels CMHT input is needed she may then do a re-referral. Of note, CMHT did assess her in 2022 and found no evidence of bipolar disorder or psychosis. Mervelee also reports that she has an eating disorder (Describes some days she cannot eat or drink anything at all), although I cannot find evidence of a formal psychiatric diagnosis of eating disorder. You mentioned โdisability issuesโ โ I am not quite sure what you are asking? She has a past medical history of type 2 diabetes and knee osteoarthritis, although I cannot see any recent documention on how the arthritis is affected her. There are no documented complications of her diabetes, it is well controlled.

The Claimant made an application for an injunction under section 1 of the Anti-Social Behaviour Crime and Policing Act 2014 on 24 June 2023. An injunction order was granted to the Claimant on 01 August 2023. Following repeat breaches of the injunction order, the Claimant made a committal application for contempt of court. The committal hearing under claim number K02CL827 took place at Clerkenwell and Shoreditch County Court on 12 June 2024 before District Judge Beecham. The Defendant attended and admitted all six allegations in the contempt application, thereby confirming breach of the injunction order dated 01 August 2023. The order was as follows: a. Penalty for committal is to be reserved to DJ Beecham, to 18 September 2024 at LOAM, with a time estimate of 2 hours. b. The injunction order dated 01.08.2023 is extended on the same terms to the final determination of the penalty on the committal application or further order. c. Personal service of this order is dispensed with, the Defendant having been in court and having been informed that the injunction would be extended. As the sentencing hearing is on 18 September 2024, the Injunction Order is currently due to expire then. We humbly request the Court to extend the injunction order in place for a further 2 years. Please find attached a copy of the Affidavit of Duvaraka Balachandran in support of the Claimant’s application.

I am employed as a Paralegal at Devonshires who are instructed on behalf of the Claimant in respect of these proceedings.

I make this affidavit as the Defendant continues to engage in anti-social behaviour in breach of the Injunction Order dated 01 August 2023. Committal hearing
On 09 July 2024, a hearing was listed before District Judge Beecham, whereby the Defendant admitted to all the allegations in the Claimantโs contempt application. In light of this, an order was made on the following terms: a. Penalty for committal is to be reserved to DJ Beecham, to 18 September 2024 at 1OAM, with a time estimate of 2 hours. b. The injunction order dated 01.08.2023 is extended on the same terms to the final determination of the penalty on the committal application or further order. c. Personal service of this order is dispensed with, the Defendant having been in court and having been informed that the injunction would be extended.
Breach of Injunction Order
On 30 August 2024, our Marketing Manager at Devonshires highlighted that the Defendant had again posted a blog referring to my colleague Narin Masera, Devonshires and the Claimant, in breach of her Injunction Order. A link to the blog was sent to our Marketing Manager via PR firm who track the blog for Devonshires. The blog appeared to have been posted online on 30 August 2024 and as at the date of the signing of this affidavit, is still online. There is now shown to me on page 1 -20 of Exhibit โDB1โ a true copy of a marked up pdf version of the online blog dated 30 August 2024 and showing all breaches of the injunction dated 01 August 2023.

They advised there were no repairs reported with regards to a broken radiator in the last six months but there have been repairs raised regarding the heating not working. A heating engineer attended on the 27th October 2022 and advised there was nofault found and gave you a demonstration on how to use the Honeywell wireless room stat. He also wrote down instructions on how to operate it. I therefore deem this part of your complaint not upheld. I reviewed our contractor, MCPโs portal to investigate the repairs reported regarding your front door. There are no reports in the past year, of a repair being raised regarding a fault to the lock on the front door. I therefore deem this part of the complaint not upheld. Oninvestigating the communication between H4W and yourself, I was unable to find evidence that demonstrated we had been clear with a decision regarding the tree removal. I can confirm that it is the residentโs responsibility to maintain the garden. This will include the removal of a tree. Wearesorry we were not clear in our communication and for the delay in responding back to you. This section of the complaint is upheld. It was reported on the 23rd February 2022 a repair to the gate and fence was required. A make safe was carried out but inline with our policy, it is the residentโs responsibility to repair/replace a fence that divides two gardens. After reviewing the photos held on the contractorโs portal that the fence in question divides two gardens. We therefore are unable to fulfil your request by repairing/replacing the fence. Onthe 22nd May 2022 it was reported to our repairs contractor, the communal door had a smashed pane of glass. A board up was completed to the broken glass on the 22/05/2022. The glass was replaced on the 18th July 2022 which is outside of our standard timescales. I therefore deem this section of the complaint upheld. I have requested an anti-social behaviour case is opened on your behalf. My colleague from the complaints department will contact you with the case reference number. I was unable to find any evidence of a complaint being logged relating to the above issues you have raised with us. Therefore, I am unable to review the complaint handling. I amsorry you had to report a staff member terminating your call when you contacted us. We take reports of poor conduct by our inhouse staff and contractors very seriously. We appreciate that you found this experience unpleasant, and we assure you that it was not representative of the services we aim to deliver. This concludes Stage 1 of our internal complaints process. Prepared breakfast, ate and commented about the sourness of the last Satsuma I sucked. I was stood over the sink washing up when I felt this sharp PAIN between my breast. I was alerted when there was another in quick succession & I was doubled over the sink. Told Tom & he said to take deep breathes in/out. Contemplating whether to call the GP or go classes. Felt some relief so went to classes. Was aware of the niggling pains in the pit of my stomach so decided to visit A&E. Called Tom to let him know what was happening. Got assessed & had to do ECG, which was done by a Trainee Nurse from Manchester. Such show of professionalism, making me reassured the NHS is in the same hands of the Future Generations of Health Care Professionals. I waited & waited… Tom showed up & I keep asking when is my turn. I was 2nd in line for hours & then I was 1st… I waited & waited… When I am ANXIOUS there are certain functions I can’t do – eating is one such. Luckily I have Foodstuff in my bag because TOM came out without eating anything much. He admonished me for saying he didn’t have to come out…I was just about to go back to the Nurse who thanked me for my patience – Yes for the Psychopaths at that Organisation that labelled Mervelee Myers, I do know when to wear my many Hats – Corporative, Professional, Assertive, etc., etc… The doctor came to get me as I got up to go back to the Nurse. Maybe 1 of my son’s age Kevin Murray & Valdin Legister , but I did not cross those boundaries by asking. He was surprised at the long wait, by this time I was doing 5+ hours. He told me to blame Jeremy Hunt. So now I’ll have to go back to the drawing board writing more Open Letters to government, etc, etc… As I told the doctor I am a Social Media Advocate to change the SYSTEMS that are making the vulnerable Scapegoats & Victims.My tests are positive & doctor was surprised at my Healthy Status despite my Deficits & Limitations. Got home from Nursery World Show & Mass Tom asked me for the Claw Bar he bought here. In the end I heard about the story of the bloke who decided to call my Hubby “THE CUNT” for closing the outer door. From I moved here these Vermin have pestered & provoked me no ends? At all hours of the day or night my doorbell will be ringing & they’d think nutn of playing loud music all night. Then 1 morning when it was a different bloke, Tom simple said to the bloke that I have to get sum rest to go to work…. This set off a chain of events that I was not prepared for at all. The bloke leaned into my door way, pointed his finger in Tom’s faced & threatened him, claiming Tom kissed his teeth at him. Believe me that was the 1st time I was knowing that in the UK kissing Ur teeth was a Criminal Act? Yesterday the corrosion had started b4 day when they rang my bell to get in. B4 I left home these 2 blokes keep going out & leaving the door open, then come back ringing my bell as it can be heard in the other flat upstairs. Because Tom closed the door after they went out, the bloke came back in & made the remarks for T to hear. In the end T took out the battery out of the bell, so if any1 is coming to my house. Plz call to let me know b4 hand. I will now have to go put that CB out of reach of TOM because I have enough Worry on my plate to be worrying about what he might decide to get up to if he is PROVOKED? I told T we are Foreigners & no matter what we will remain Foreigners in their eyes. Despite what any1 says we don’t have any RIGHTS as I have proven time & time again. I went to the Nursery World Show yesterday to update some of my existing knowledge. Attended The Progress Check at Two: getting it right for the child and parents. Well the speaker Vicky Hutchin was vocal about her concerns re the dumbing down of training especially Child Development. And how practitioners are empowered to do their jobs. I have been raising these same concerns since b4 I graduated from the OU. And where did that get Me…? Labelled, stereotyped and branded as a Know It All, who knows nutn! Well time alone is the Master/Mistress in these changing times. I then attended Understanding and supporting Schemas and when I saw a different name from the 1 I’d registered with, I went to enquire ensuring I was in the right place. Julie Brierley had to replace Stella who contracted Chicken Pox. NurseryWorld Show was very educational & informative indeed. I was in a PS so used my camera to capture my Story.

Just wondering if Facebook for Windows realising that they are still terrorising me with their adverts? Facebook is responsible for giving LEYF Nurseries – #LEYF access to my account from the 18th September 2015. Up to last week Facebook tried locking me out of my account. Facebook Legal Team contacted me on behalf of Bates Wells Braithwaite. That’s bloody terrorism. Facebook I have not worked from my nervous breakdown that Voice: the union for education professionals is a party to. It’s not about the disabilities because I got support at the Maudsley Hospital. But about the 6 lines reference flagging safeguarding. And the Department for Work and Pensions – DWP threatening to issue me with criminal records.
