I was lying in bed listening to BBC Radio 4 Appeal when my attention was drawn to the mention of Parkinson disease. For me the word Parkinson was like a red flag to a raging bull because of the effects the disease had on my Family when I was younger. Therefore Parkinson is embedded in my DNA and although I am better equipped to deal with it now, before it was a nightmare. I listened to every words uttered by Gavin Hastings as he described the signs and symptoms of Parkinson as they resonated with me. I can empathised with Gavin Hasting and his family when he talked movingly about the impact on his family when his wife was diagnosed with the condition at the age of 39 years old.
I know almost everything there is to know about this dreaded Parkinson disease because of my own personal experiences from an early age. My family that is my dad and younger uncle in particular were affected by Parkinson, and this had very devastating effects on their lives and that of the families. Worse was to come for me when I discovered at the age of 20 years that the only reasons Iโd underachieved throughout my school life and until the discovery was that I have Parkinson disease. I guess I should have known earlier about the evidence staring me in the face but I didnโt.
Dad took sick and was signed off from work when I was still at Primary School and died when I was 20 years old. I helped mum with his care especially in the last years of his life because I had become a parent and a stay at home mum whilst mum took on dadโs role of main bread winner. Therefore I had all the time in the world to witness and identify the signs and symptoms of Parkinson that were manifested in my daily life. So from an early age I started experiencing some of the ailments that are associated with Parkinson disease. In later life when I realised that depression was closely linked to Parkinson, I could better understand my changing mood swings when Parkinson decided to take a hold on me. Sadly for me if I had all this knowledge that I have now at my disposal my life might have turned out differently?
I grew up being very angry about having the bad luck of the only surviving girl in the family. Only to be struck down with the same afflictions which rendered my dad useless in every sense of the word for many years of his short life. Unlike dad who had his God along with his Christian beliefs to see him through those difficult years in his life. I had no one to turn to with my dilemma, so I became a rebel and blamed dadโs God for failing us. I just could not understand how the God that dad loved, worshipped and dedicated his every existence to could allow dad to suffer the way he did? Although I had the nerves to question Godโs handling of the matter, my dadโs faith in his God never faltered. In retrospect I am more than glad that dad was steadfast in his beliefs and I am happy to say I inherited some of his traits that are priceless and make the person I am today.
By this time I have 2 children so have to stop feeling sorry for myself and get on with this business called life of fulfilling my roles and responsibilities as a single mum. So I set about being accountable by making some life changing decisions to benefit my children and myself. I was already prescribed Valium in my teens and I made my own self-diagnosis that I had inherited my dad and uncleโs condition. There was absolutely nothing I could do about it, so had to live the best way I could manage. I knew without a doubt from I was 20 years old that I have PARKINSON DISEASE and I have tried to live my life around the condition ever since.
With knowledge comes wisdom and power and Iโve benefited enormously from the enhanced knowledge over the years as I have studied and being given opportunities to change my life. But surprise of surprises anyone with my condition are opened to DISCRIMINATIONS because of ignorance. I was penalised for becoming an ADVOCATE applying knowledge to identify children and families with diverse and complex needs so they can benefit from early intervention strategies. I am still facing misunderstandings as my deep concerns are misconstrued to show me in a bad light. These days I am using my knowledge, values and beliefs to good effects hoping my forward thinking visions and aspirations will prevent anyone else going through what I went through in my school life and later.
I am striving to stay away from confrontations because I know that lack of knowledge creates ignorance. As I said when I first embarked on studies at Lambeth College and done a placement at Turney Special School โ โYou cannot tell by lookingโฆโ I know exactly what I am talking about on certain matters because I have the experiences to back up my arguments. These days instead of succumbing to the pitfalls associated with conflicts and confrontations, I spend my time writing about my Life Journeys.
I give my dadโs GOD the honour and praises for seeing me through as I struggle, because I am still a sceptic. However I am a more tolerant person and I am glad to say my condition has not deteriorated any further from when I left Jamaica over 21 years ago. I know I have all the signs and symptoms that will never go away, but I am managing. I may have pass on my condition to 1 of my children but I know advancement in science have brought hopes to persons like me who have inherited a condition like Parkinson. I am detailing my life in writings so one day my family will get the chance to see me in a different light and may began to get a clearer picture of who I am and why I did not become the person I expected to?

Thank you for your emails and for the voicemails you left me. I’ll add everything you’ve sent me to the complaint, but unfortunately it doesn’t change what I have said in my view.
When we assess a complaint we have to take all the information you gave us, as well as all the information that Barclays have given us and make a decision based on that. In this case, I cannot find any factual evidence that Barclays have your missing ยฃ1,000. So I can’t say they need to give it back or reopen your account. I am sorry about that as I know it’s very disappointing and not very helpful for you. But we are an evidence based organisation.
I know that you are caring for your husband at the moment and I don’t want to add to your stress or worry. If you don’t agree with my findings, I can ask for an ombudsman to review your case again and make sure what I have said is correct. Would you like me to do that?
You asked in one of your emails what qualifications I have. I am part of the additional support area and we do receive extra training around mental health issues and complaints where someone has had an effect on their health. I hope that helps reassure you that I have looked at everything as thoroughly as I can.

As a member of the Resident Panel, Mervelee Myers is exercising my rights as a vulnerable person who has been let down by the Housing Ombudsman Services and Mr. Richard Blakeway in particular to the point where after attending Court on Wednesday 18th September 2024, I came away feeling SUICIDAL. After leaving Court I attended my appointment with Dennis Kamara and was in time to hear him talking with a female on the phone. The purpose of the talk was Dennis was arranging support for me with Mental Health Older Adults Service 02032286920. After leaving the appointment I had to take my “Marriage Certificate” to the Job Centre to get it scanned to send off to get Bereavement Support Payment. I visited a friend of mine and was too SCARED to come back to my home where I no longer feel SAFE. I sleep on his sofa and leave early next morning. I have not eaten a proper meal in days and having DIABETES does not help my situation. I had to call the Police again when my neighbour started shouting abuse at me and threatening to come down to SORT me out. CAD 7435/16SEP24 and I have an appointment to be interviewed on the 22/09/2024. I have been living like a PRISONER at Alma Grove London SE1 5PY since I was forced to barricade myself in on Christmas eve 2022 for over an hour. The Safer Neighbourhood Team was supposed to contact me from then, am still waiting. Let me share more recent information about my engagement with the Metropolitan Police that will expose “A New Met for London” as a cover for GASLIGHTING and MISOGYNY in breach of the Equality Act 2010 Protected Characteristics in the UK in 2024. No Blacks No Dogs No Irish. CAD 5710/12SEP/24 and CAD 5309/0Aug24 and CAD 3200/19May24 and CAD5569/17May24 and CAD 4631/17May24. Make Things Right Campaign: Mervelee Myers wants to share information about the Systemic Discrimination and Miscarriages of Justice starting at Kings College Hospital NHS Foundation Trust 2003-2008 when I had the first nervous breakdown after the death of my brother wit colon cancer. To 2024 when my husband was neglected on BYRON ward and on the 6th April 2024 after attending the Windrush Vigil in Brixton Nurse Betty called Security I was ABUSING STAFF. I was removed from my husband’s bedside and ended in A&E at St Thomas’ Hospital. I never saw my husband again until he died on the 8th and they waited 6 hours to inform me. Since LEYF CEO sanctions the DISCRIMINATION and labels me Unprofessional, Uncoorporative, Rude, Intimidating, Confrontational, Aggressive – Lacking Empathy (UURICA-LE) after I fronted OFSTED at BIB in September 2014 and was given the CEO Long Service Award. Let me put the Government in the spotlight. In 2011 I meet Michael Gove at LEYF Annual Conference. I represented LEYF at a Consultation with the Communities Secretary Eric Pickles. I wrote an Open Letter to PM David Cameron in October 2015. I write another Open Letter to PM Theresa May in 2017 and take out a PETITION that was rejected. This was the same year when Winsome Duncan sent me 3 REVIEWS on LEYF website about the abuses at LEYF Nurseries. By this time I was able to access LEYF websites. Mervelee Myers respond to put Devonshires Solicitors LLP that I am aware of the proceedings and trying to keep up.
The Claim
Let me begin by starting with the letter that was sent from Devonshires Solicitors LLP Narin Masera in July 2023 after Housing for Women letter about mediation came after the date, I was supposed to contact them about the mediation. Before receiving the letter from Narin Masera about possession, I had no knowledge about Devonshires Solicitors LLP.
Without Prejudice as a Lay Person who has been forced to represent myself from the first time, I experienced “Systemic Discrimination” after the death of my brother with “Colon Cancer” I will use the “Equality Act 2010 Protected Characteristics” to address this claim.
Improper use of the Pilot
I will repeat as a “Lay Person” I have to rely on myself to get justice from the Injustices because I was a participant in Dr. Maria Hudson’s research paper “The Experience of Multiple Discrimination” for the Policy Studies Institute recommended to ACAS. Please refer to the BUNDLE of K02CL827 at page 320 for a sample of an email from rattynem@btinternet.com to Maria Hudson, Policy Studies Institute, 50 Hanson Street, London W1W 6UP. Therefore, I will ask Devonshires Solicitors LLP like what I did when I called Narin Masera on the day I was taking him to look about the “Power of Attorney” with Mimi Owusu who introduced us to Amphlett Lissimore Solicitors Greystoke House, 80-86 Westow Street, London SE19 3AF. I called in good faith the number that was on the letter, hoping to make Devonshires Solicitors LLP aware of the reputation of h4W.
I will repeat I spent the past 10 years trying to get my life back on track. For Devonshires Solicitors LLP to join H4W in breaching the Equality Act 2010 Protected Characteristics by the GASLIGHTING and MISOGYNY must be highlighted.
All I can say on this matter is I have used the CCMCC before making claims and faced more miscarriages of justice. Hence I know the names of some of the District Judges involved in this case. For example, DJ Sterlini who labelled me a “VIOLENT NUISANCE” on Emancipation Day 2023 strike out my claims with Winsome Duncan after it reached “Telephone Conciliation”. DJ Bell who requested for me to do the “Psychiatric Assessment” sent 7 of my claims from County Court at Clerkenwell and Shoreditch to Central London County Court including Barrister Ryan Clement that judgement was passed for him to pay the ยฃ9455.00. I am still waiting to be paid, but 4 years after Court Enforcement failed to collect my money. They are sending me threats…
Here again is proof of how Mervelee Myers is the victim of the Judiciary of England and Wales, the Criminal Justice System and the Crown Prosecution Service when unscrupulous Devonshires Solicitors LLP decide to use Mervelee Myers VULNERABILITY against me.
Lack of Merits of the Claim
As a person with “Mental and Physical Impairments” whose stories are documented online before Devonshires Solicitors LLP got HMCTS to gag me with threats of IMPRISONMENT and EVICTION via Contempt of Court, I give up my rights in order that I don’t become a victim like Nelson Mandela who was made a CRIMINAL for his thoughts and ideas. Your Client knows well what they are doing to tenants. So does the Housing Ombudsman Service Richard Blakeway. For Clarity may I advice you to look at the BUNDLE pages 217-228 of Mimi Owusu’s “Witness Statement.
Mervelee Myers will not argue about what the Court has done. And Nurse Betty called WHITE SECURITY to remove me from his bedside on the 6th April 2024 after I attended the Windrush Vigil. For your information Trina Philbert provoked me on the 4th April 2024 after the HEARING. Some of the posters were that of the Rev Rose Hudson-Wilkin the first Female Bishop of the COE whose grandmother and my mother were related. I had poster of Jessie Lloyd an Australian Aborigine who Mr. Tony Cealy introduce me to and I am listed as the COMPOSER for Border Crossings SONGSTREET App.
I was not the one engaging in any such behaviours by the way. I refuse to engage with the MALICIOUS arguments you have decided to put on paper. Instead ask your client to stop the coercive control of Deborah Agnes Gilchrist to target me. Please get her the help she needs to stop doing to me all listed above and more.

Housing Ombudsman Service Responsible For Mervelee Myers Scared Of Acting On Suicidal Thoughts.
As a member of the Resident Panel, Mervelee Myers is exercising my rights as a vulnerable person who has been let down by the Housing Ombudsman Services and Mr. Richard Blakeway in particular to the point where after attending Court on Wednesday 18th September 2024, I came away feeling SUICIDAL. After leaving Court I attended my appointment with Dennis Kamara and was in time to hear him talking with a female on the phone. The purpose of the talk was Dennis was arranging support for me with Mental Health Older Adults Service 02032286920. After leaving the appointment I had to take my “Marriage Certificate” to the Job Centre to get it scanned to send off to get Bereavement Support Payment. I visited a friend of mine and was too SCARED to come back to my home where I no longer feel SAFE. I sleep on his sofa and leave early next morning. I have not eaten a proper meal in days and having DIABETES does not help my situation. I had to call the Police again when my neighbour started shouting abuse at me and threatening to come down to SORT me out. I have been living like a PRISONER at Alma Grove London SE1 5PY since I was forced to barricade myself in on Christmas eve 2022 for over an hour. I have another Reference 2024000031 representing the 23 years of HATE CRIMES against me and how I managed to live with an ALCOHOLIC who spent time in prison for DRUNK DRIVING. I refuse from breaching the INJUNCTION so am IMPRISONED and EVICTED before I get to submit the RESEARCH FINDINGS about Housing for Women and Devonshires Solicitors LLP plots. The plots with the Judiciary of England and Wales and Criminal Justice System and Crown Prosecution Services with the continuing MISCARRIAGES of JUSTICE. Housing Ombudsman Service Responsible For Mervelee Myers Scared Of Acting On Suicidal Thoughts.
As a member of the Resident Panel, Mervelee Myers is exercising my rights as a vulnerable person who has been let down by the Housing Ombudsman Services and Mr. Richard Blakeway in particular to the point where after attending Court on Wednesday 18th September 2024, I came away feeling SUICIDAL. After leaving Court I attended my appointment with Dennis Kamara and was in time to hear him talking with a female on the phone. The purpose of the talk was Dennis was arranging support for me with Mental Health Older Adults Service 02032286920. After leaving the appointment I had to take my “Marriage Certificate” to the Job Centre to get it scanned to send off to get Bereavement Support Payment. I visited a friend of mine and was too SCARED to come back to my home where I no longer feel SAFE. I sleep on his sofa and leave early next morning. I have not eaten a proper meal in days and having DIABETES does not help my situation. I had to call the Police again when my neighbour started shouting abuse at me and threatening to come down to SORT me out. CAD 7435/16SEP24 and I have an appointment to be interviewed on the 22/09/2024. I have been living like a PRISONER at Alma Grove London SE1 5PY since I was forced to barricade myself in on Christmas eve 2022 for over an hour. Make Things Right Campaign: Mervelee Myers wants to share information about the Systemic Discrimination and Miscarriages of Justice starting at Kings College Hospital NHS Foundation Trust 2003-2008 when I had the first nervous breakdown after the death of my brother wit colon cancer. To 2024 when my husband was neglected on BYRON ward and on the 6th April 2024 after attending the Windrush Vigil in Brixton Nurse Betty called Security I was ABUSING STAFF. I was removed from my husband’s bedside and ended in A&E at St Thomas’ Hospital. I never saw my husband again until he died on the 8th and they waited 6 hours to inform me. Since LEYF CEO sanctions the DISCRIMINATION and labels me Unprofessional, Uncoorporative, Rude, Intimidating, Confrontational, Aggressive – Lacking Empathy (UURICA-LE) after I fronted OFSTED at BIB in September 2014 and was given the CEO Long Service Award. Let me put the Government in the spotlight. In 2011 I meet Michael Gove at LEYF Annual Conference. I represented LEYF at a Consultation with the Communities Secretary Eric Pickles. I wrote an Open Letter to PM David Cameron in October 2015. I write another Open Letter to PM Theresa May in 2017 and take out a PETITION that was rejected. This was the same year when Winsome Duncan sent me 3 REVIEWS on LEYF website about the abuses at LEYF Nurseries. By this time I was able to access LEYF websites. I am a GP working at the surgery Mervelee is registered at. Please note that my previous clinical interaction with Mervelee consists of 3x 10minute consultations since 2017. Please note that time allocated by the surgery for completion of this letter was 20minutes, so I am note able to be as thorough as I would have liked, and I am also not sure that I am going to be able to add much to the thorough psychiatric report that was completed in April. You ask for specific mental health conditions โ she was diagnosed with anxiety and depression several years ago (first coded in 2018). There was a query of bipolar disorder made by my colleague Dr Evans due to paranoid behaviour, pressure of speech, labile mood (please note formal bipolar disorder diagnoses are not generally made by GPs, this is something our psychiatry colleagues do). A CMHT referral was done but rejected, they asked she see the mental health nurse first but it appeared this never happened, although she does now have an appointment at the end of August with the mental health nurse at the surgery โ if the nurse feels CMHT input is needed she may then do a re-referral. Of note, CMHT did assess her in 2022 and found no evidence of bipolar disorder or psychosis. Mervelee also reports that she has an eating disorder (Describes some days she cannot eat or drink anything at all), although I cannot find evidence of a formal psychiatric diagnosis of eating disorder. You mentioned โdisability issuesโ โ I am not quite sure what you are asking? She has a past medical history of type 2 diabetes and knee osteoarthritis, although I cannot see any recent documention on how the arthritis is affected her. There are no documented complications of her diabetes, it is well controlled.

The Claimant made an application for an injunction under section 1 of the Anti-Social Behaviour Crime and Policing Act 2014 on 24 June 2023. An injunction order was granted to the Claimant on 01 August 2023. Following repeat breaches of the injunction order, the Claimant made a committal application for contempt of court. The committal hearing under claim number K02CL827 took place at Clerkenwell and Shoreditch County Court on 12 June 2024 before District Judge Beecham. The Defendant attended and admitted all six allegations in the contempt application, thereby confirming breach of the injunction order dated 01 August 2023. The order was as follows: a. Penalty for committal is to be reserved to DJ Beecham, to 18 September 2024 at LOAM, with a time estimate of 2 hours. b. The injunction order dated 01.08.2023 is extended on the same terms to the final determination of the penalty on the committal application or further order. c. Personal service of this order is dispensed with, the Defendant having been in court and having been informed that the injunction would be extended. As the sentencing hearing is on 18 September 2024, the Injunction Order is currently due to expire then. We humbly request the Court to extend the injunction order in place for a further 2 years. I am employed as a Paralegal at Devonshires who are instructed on behalf of the Claimant in respect of these proceedings.

I make this affidavit as the Defendant continues to engage in anti-social behaviour in breach of the Injunction Order dated 01 August 2023. Committal hearing
On 09 July 2024, a hearing was listed before District Judge Beecham, whereby the Defendant admitted to all the allegations in the Claimantโs contempt application. In light of this, an order was made on the following terms: a. Penalty for committal is to be reserved to DJ Beecham, to 18 September 2024 at 1OAM, with a time estimate of 2 hours. b. The injunction order dated 01.08.2023 is extended on the same terms to the final determination of the penalty on the committal application or further order. c. Breach of Injunction Order
On 30 August 2024, our Marketing Manager at Devonshires highlighted that the Defendant had again posted a blog referring to my colleague Narin Masera, Devonshires and the Claimant, in breach of her Injunction Order. A link to the blog was sent to our Marketing Manager via PR firm who track the blog for Devonshires. The blog appeared to have been posted online on 30 August 2024 and as at the date of the signing of this affidavit, is still online. There is now shown to me on page 1 -20 of Exhibit โDB1โ a true copy of a marked up pdf version of the online blog dated 30 August 2024 and showing all breaches of the injunction dated 01 August 2023.

I met with Ms Myers in clinic at Marina House today alongside her allocated care coordinator, Hawa Oweh (CPN).
Background
Ms Myers is a 65-year-old female, not previously known to the Southwark Older Adult CMHT, however previously under working age mental health services. Ms Myers was referred to our team by Dennis Kamara, a mental health nurse in primary care. He requested for an assessment of diagnosis and treatment options, with information that Ms Myers has a long history of low mood and anxiety with some previous underlying trauma and recent stressors including her housing association taking her to court and ongoing disputes with her neighbours.
The following information about Ms Myersโs mental health background is taken from available electronic mental health records.
Ms Myersโ mental health records date back to 2017 where there is a Merlin report from police with concern about Ms Myers struggling with her mental health following a tribunal, with also a mention of her receiving previous support from the Maudsley. This however doesnโt appear to have been followed by input from secondary mental health care. In 2021 there are a number of contacts with the crisis line and referrals to CMHT (working age), outlining concerns about low mood, anxiety and suicidal thoughts for the past 7 years since her mother passed away. Other contributory factors noted included the death of a neighbour the previous year and being charged with criminal damage and attending court in April 2021. Ms Myers had also spoken about the death of her brother and developing depression and an eating disorder as a result of discrimination at work. Another referral to CMHT in 2021 reportedly from Southwark council outlined concerns of Ms Myers feeling police were being sent to kill her. There was also a phone call from a mental health practitioner from the University of East London (student wellbeing service) expressing concerns about Ms Myersโ mental health and possible bizarre and paranoid behaviour.
In 2022 Ms Myers was assessed following a GP referral by the Southwark Assessment & Liaison Team. This assessment noted mental health difficulties appearing to commence in 2014 when Ms Myers was reportedly persecuted in her place of work, having worked as an early yearโs practitioner. It was reported that Ms Myers was being accused by her nursery in Southwark of being โuncooperative, rude and confrontationalโ, and was under investigation, and then left due to ill health. Following this, she received 12 sessions of Cognitive Behavioural Therapy (CBT) at the Maudsley. Following this, Ms Myers had reported not being able to gain paid employment, but had worked voluntarily in mental health research, been involved in various organisations and online podcasts, and was also featured in a book about Windrush. Ms Myers had reported an incident of the police in 2021 attending her home and beating her, and that she now has a criminal record and has been turned down for jobs due to her criminal record check (CRB). She had reported feeling she has been targeted by certain people she used to work with and there was a pattern. The impression of this review was that there was not any clear evidence of depression or psychosis, that past events have been very stressful and that Ms Myers could benefit from further talking therapy. She was then referred to IAPT.
In 2023 and going into 2024 there were a number of referrals to CMHT, due to concerns about Ms Myersโ mental health. There was reference of Ms Myersโ housing association seeking legal action due to antisocial behaviour. In December 2023 Ms Myers was reported to have called 111 herself reporting feeling hopeless about the threat of eviction from her home and having ongoing issues with her neighbours. In March 2023 Ms Myers was brought to A&E by the London Ambulance Service reporting worsening mental health. No evidence of thought disorder or psychosis observed and GP was advised to review Ms Myers and consider a referral to mental health services.
In September 2024 Ms Myers was referred to the Southwark Older Adult CMHT by Dennis Kamara, mental health nurse in primary care. There were concerns related to her behaviour and posting references to suicide in her social media. Ms Myers was seen by Dennis in primary care alongside her son Kevin who had travelled from Jamaica where he lives. There were reported issues with housing, neighbours and family. The housing association has reportedly taken an injunction against her and she is at risk of eviction and the case is currently in court. Her son had reported the reason for this being Ms Myers putting up inappropriate material online about the council and refusing to take it down. Her son had mentioned Ms Myers storing urine for months to throw it at her employerโs property and putting up comments on social media about feeling suicidal, which Ms Myers admitted to, however, stated she would never do this due to her Christian faith. There was also concern from Ms Myers about a camera being placed in the shared communal doorway by her neighbours, that it initially faced towards the street however now has been turned directly at her. She had reportedly denied feeling the neighbours watch her when she is in her flat however, the only issue is when she is coming through the communal front door.
Recent progress
Ms Myers was initially assessed by our team on 08/10/24 at home by Joseph Mukuba, Community Psychiatric Nurse. Following this, I decided to assess Ms Myers myself to help come to a psychiatric diagnosis and consider treatment options.
I have subsequently reviewed Ms Myers in clinic on 16/10/24. During this assessment Ms Myers described to me in detail her background history to date, including the trauma in her upbringing (physical abuse from mother, father dying when she was a young age), and her first marriage to a controlling husband resulting in domestic violence. Ms Myers talked about losing her mother 10 years ago, and around the same time being forced out of working as a nursery nurse and following this receiving therapy. Ms Myers talked about the issues with her neighbours persecuting her, which have worsened over the years, and her recent bereavement of her husband a few months ago.
On 23/10/24 Ms Myers contacted 111 and LAS due to feeling in โcrisisโ after her grandniece left having stayed with her without saying goodbye. I visited Ms Myers at home on the 25/10/24. During this visit, Ms Myers described in more detail the persecution she reports experiencing from her neighbours, whom she believes are trying to kill her, leading to her feeling unsafe at home. She reported since her husband passed away, she has been more careful in locking her front door and that if she didnโt do that โI would be deadโ. Ms Myers showed me the doorbell camera her neighbour set up on the outside of the shared front door, and how she believes the neighbour is using this to monitor her coming in and out of her home. Ms Myers talked about her neighbour kicking on her front door every day to provoke her and her belief that her neighbours want to try to provoke her into confronting them, which would then give them an excuse to harm and kill her. Ms Myers has explained to me how she believes other neighbours are also siding against her and that she also feels under threat from the police and housing association, all working together against her.

I summarised the purpose of review to complete the assessment following the referral to our team from Dennis Kamara, primary care mental health nurse.
I asked Ms Myers how she has been since we last met. She talked about the housing association not coming yet to fix her toilet seat, as well as the mould on her kitchen door and the damp in her windowsill. She reported attending her drumming group and doing a performance and spoke positively about this. She brought up about her neighbours continuing to try to provoke her since we last met. Ms Myers spoke about the neighbours โstompingโ on the floor loudly to provoke her. Ms Myers agreed that she believed the neighbours were trying to kill her. I recalled her previous statement to me that she believed the neighbours were trying to provoke her so that she would in turn confront them, and this would give them the excuse to try to kill her. Ms Myers accepted this is what she believed. She stated she would not attack them herself and is trying to keep to herself. Ms Myers said recently she stayed at a friendโs house overnight as she didnโt feel safe to return home. I asked about the camera. Ms Myers continued to report that she believed the camera on the doorbell was there to monitor her movements. She said she is trying not to think about this. She stated being more frustrated about her doorbell always being rung.
Ms Myers made a statement about never wanting to โgive upโ. I enquired about whether she experiences suicidal thoughts. Ms Myers stated this has been misconstrued and referenced a poster or piece of literature she was pictured in, that had the quote about people being unemployed, being at risk of suicide, and said this quote was then attached to her. Ms Myers did admit to having last had a suicidal thought in August this year, when her son was visiting her and she talked about her door being kicked. She didnโt report having experienced any plans or intent to act on this. Ms Myers stated there was no chance that she would come to harm via her own means. She has previously stated her Christian faith as a protective factor.
I summarised to Ms Myers what she had explained to me over the last 3 meetings leading to my opinion about her mental health diagnosis. This included Ms Myersโ description of her earlier life and trauma she experienced, the domestic violence in her first marriage which led to her separating and being saved by the housing situation, the loss of her mother 10 years ago around the same time as stopping working as a nursery nurse. Then over recent years with the increasingly difficult situation with her neighbours, beliefs that they are provoking her by monitoring her movements, trying to kill her and banging on her door, leading to her receiving a possession order which may potentially result in her being evicted and a possible custodial sentence due to being in contempt of court. Then more recently the stress of losing her husband. I explained to Ms Myers my impression that as a result of the build up of stress over these years that her beliefs and experiences about the neighbours could be explained by her being unwell with paranoia and psychosis. I explained that this is a condition that we could provide treatment for, including both antipsychotic medication, and psychosocial support, including psychology for psychosis. Ms Myers said in response to this that she had experienced some paranoia in the past. She said she accepted this was my explanation of opinion about her mental health. Ms Myers said she was willing to try medication if it would help. Ms Myers explained she had recently been to her new GP for vaccinations and blood tests, I advised I would check the results of these and then give her a call next week to see whether she would like to go ahead with starting antipsychotic medication. She agreed to this and accepted a patient information leaflet about risperidone. She confirmed having our contact numbers in case needing to get in touch in between appointments. Hawa confirmed she would make an appointment with Ms Myers when she had started on her medication to monitor how she is doing. We also discussed social activities and community groups, Ms Myers knew of Time & Talents and didnโt want to attend there, however appeared keener in Blackfriars.
Mental state examination
Ms Myers presented on time, she was well kempt, wearing a diabetes charity t-shirt, engaged well, reasonable rapport established, maintained eye contact, speech slightly pressured however, interruptible and voice not raised, mood reported as well and objectively appeared euthymic with reactive affect. Mild thought disorder evident with circumstantial responses. Describing persecutory delusional thoughts about neighbours trying to kill her and monitoring her movements. No reported auditory hallucinations and didnโt appear to be responding to unseen stimuli. Appeared well orientated to time and place. Insight partial however took on board opinion about diagnosis and agreed to consider treatment options explained.
Care Plan
I have provided written information for Ms Myers for an antipsychotic medication – risperidone. I advised I will check Ms Myerโs most recent blood tests with her GP and call her following this to see whether she would like to start risperidone or not.
I will send this assessment letter to Mrs Myers, and she is then able to share this with her solicitor for her current court case.
Ms Myers is assigned a care coordinator in our team โ Hawa Oweh, CPN โ who will monitor Ms Myersโ mental state and risks and support with psychosocial interventions.
Hawa Oweh has agreed to make contact with the housing association to support Ms Myers with fixing her toilet seat and other house matters needing repair.
Ms Myers confirmed having contact numbers for people to call, including the Samaritans, our team number during working hours (0203 228 2240) and the SLAM crisis line out of hours (0800 731 2864).

I met with Ms Myers in clinic at Marina House today alongside her allocated care coordinator, Hawa Oweh (CPN).
Background
Ms Myers is a 65-year-old female, not previously known to the Southwark Older Adult CMHT, however previously under working age mental health services. Ms Myers was referred to our team by Dennis Kamara, a mental health nurse in primary care. He requested for an assessment of diagnosis and treatment options, with information that Ms Myers has a long history of low mood and anxiety with some previous underlying trauma and recent stressors including her housing association taking her to court and ongoing disputes with her neighbours.
The following information about Ms Myersโs mental health background is taken from available electronic mental health records.
Ms Myersโ mental health records date back to 2017 where there is a Merlin report from police with concern about Ms Myers struggling with her mental health following a tribunal, with also a mention of her receiving previous support from the Maudsley. This however doesnโt appear to have been followed by input from secondary mental health care. In 2021 there are a number of contacts with the crisis line and referrals to CMHT (working age), outlining concerns about low mood, anxiety and suicidal thoughts for the past 7 years since her mother passed away. Other contributory factors noted included the death of a neighbour the previous year and being charged with criminal damage and attending court in April 2021. Ms Myers had also spoken about the death of her brother and developing depression and an eating disorder as a result of discrimination at work. Another referral to CMHT in 2021 reportedly from Southwark council outlined concerns of Ms Myers feeling police were being sent to kill her. There was also a phone call from a mental health practitioner from the University of East London (student wellbeing service) expressing concerns about Ms Myersโ mental health and possible bizarre and paranoid behaviour.
In 2022 Ms Myers was assessed following a GP referral by the Southwark Assessment & Liaison Team. This assessment noted mental health difficulties appearing to commence in 2014 when Ms Myers was reportedly persecuted in her place of work, having worked as an early yearโs practitioner. It was reported that Ms Myers was being accused by her nursery in Southwark of being โuncooperative, rude and confrontationalโ, and was under investigation, and then left due to ill health. Following this, she received 12 sessions of Cognitive Behavioural Therapy (CBT) at the Maudsley. Following this, Ms Myers had reported not being able to gain paid employment, but had worked voluntarily in mental health research, been involved in various organisations and online podcasts, and was also featured in a book about Windrush. Ms Myers had reported an incident of the police in 2021 attending her home and beating her, and that she now has a criminal record and has been turned down for jobs due to her criminal record check (CRB). She had reported feeling she has been targeted by certain people she used to work with and there was a pattern. The impression of this review was that there was not any clear evidence of depression or psychosis, that past events have been very stressful and that Ms Myers could benefit from further talking therapy. She was then referred to IAPT.
In 2023 and going into 2024 there were a number of referrals to CMHT, due to concerns about Ms Myersโ mental health. There was reference of Ms Myersโ housing association seeking legal action due to antisocial behaviour. In December 2023 Ms Myers was reported to have called 111 herself reporting feeling hopeless about the threat of eviction from her home and having ongoing issues with her neighbours. In March 2023 Ms Myers was brought to A&E by the London Ambulance Service reporting worsening mental health. No evidence of thought disorder or psychosis observed and GP was advised to review Ms Myers and consider a referral to mental health services.
In September 2024 Ms Myers was referred to the Southwark Older Adult CMHT by Dennis Kamara, mental health nurse in primary care. There were concerns related to her behaviour and posting references to suicide in her social media. Ms Myers was seen by Dennis in primary care alongside her son Kevin who had travelled from Jamaica where he lives. There were reported issues with housing, neighbours and family.
