I am a GP working at the surgery Mervelee is registered at. Please note that my previous clinical interaction with Mervelee consists of 3x 10minute consultations since 2017. Please note that time allocated by the surgery for completion of this letter was 20minutes, so I am note able to be as thorough as I would have liked, and I am also not sure that I am going to be able to add much to the thorough psychiatric report that was completed in April. You ask for specific mental health conditions โ she was diagnosed with anxiety and depression several years ago (first coded in 2018). There was a query of bipolar disorder made by my colleague Dr Evans due to paranoid behaviour, pressure of speech, labile mood (please note formal bipolar disorder diagnoses are not generally made by GPs, this is something our psychiatry colleagues do). A CMHT referral was done but rejected, they asked she see the mental health nurse first but it appeared this never happened, although she does now have an appointment at the end of August with the mental health nurse at the surgery โ if the nurse feels CMHT input is needed she may then do a re-referral. Of note, CMHT did assess her in 2022 and found no evidence of bipolar disorder or psychosis. Mervelee also reports that she has an eating disorder (Describes some days she cannot eat or drink anything at all), although I cannot find evidence of a formal psychiatric diagnosis of eating disorder. You mentioned โdisability issuesโ โ I am not quite sure what you are asking? She has a past medical history of type 2 diabetes and knee osteoarthritis, although I cannot see any recent documention on how the arthritis is affected her. There are no documented complications of her diabetes, it is well controlled.

The Claimant made an application for an injunction under section 1 of the Anti-Social Behaviour Crime and Policing Act 2014 on 24 June 2023. An injunction order was granted to the Claimant on 01 August 2023. Following repeat breaches of the injunction order, the Claimant made a committal application for contempt of court. The committal hearing under claim number K02CL827 took place at Clerkenwell and Shoreditch County Court on 12 June 2024 before District Judge Beecham. The Defendant attended and admitted all six allegations in the contempt application, thereby confirming breach of the injunction order dated 01 August 2023. The order was as follows: a. Penalty for committal is to be reserved to DJ Beecham, to 18 September 2024 at LOAM, with a time estimate of 2 hours. b. The injunction order dated 01.08.2023 is extended on the same terms to the final determination of the penalty on the committal application or further order. c. Personal service of this order is dispensed with, the Defendant having been in court and having been informed that the injunction would be extended. As the sentencing hearing is on 18 September 2024, the Injunction Order is currently due to expire then. We humbly request the Court to extend the injunction order in place for a further 2 years. I make this affidavit as the Defendant continues to engage in anti-social behaviour in breach of the Injunction Order dated 01 August 2023. Committal hearing
On 09 July 2024, a hearing was listed before District Judge Beecham, whereby the Defendant admitted to all the allegations in the Claimantโs contempt application. In light of this, an order was made on the following terms: a. Penalty for committal is to be reserved to DJ Beecham, to 18 September 2024 at 1OAM, with a time estimate of 2 hours. b. The injunction order dated 01.08.2023 is extended on the same terms to the final determination of the penalty on the committal application or further order. c. Personal service of this order is dispensed with, the Defendant having been in court and having been informed that the injunction would be extended.
Breach of Injunction Order
On 30 August 2024, our Marketing Manager at Devonshires highlighted that the Defendant had again posted a blog referring to my colleague Narin Masera, Devonshires and the Claimant, in breach of her Injunction Order. A link to the blog was sent to our Marketing Manager via PR firm who track the blog for Devonshires. The blog appeared to have been posted online on 30 August 2024 and as at the date of the signing of this affidavit, is still online. There is now shown to me on page 1 -20 of Exhibit โDB1โ a true copy of a marked up pdf version of the online blog dated 30 August 2024 and showing all breaches of the injunction dated 01 August 2023.

I met with Ms Myers in clinic at Marina House today alongside her allocated care coordinator, Hawa Oweh (CPN).
Background
Ms Myers is a 65-year-old female, not previously known to the Southwark Older Adult CMHT, however previously under working age mental health services. Ms Myers was referred to our team by Dennis Kamara, a mental health nurse in primary care. He requested for an assessment of diagnosis and treatment options, with information that Ms Myers has a long history of low mood and anxiety with some previous underlying trauma and recent stressors including her housing association taking her to court and ongoing disputes with her neighbours.
The following information about Ms Myersโs mental health background is taken from available electronic mental health records.
Ms Myersโ mental health records date back to 2017 where there is a Merlin report from police with concern about Ms Myers struggling with her mental health following a tribunal, with also a mention of her receiving previous support from the Maudsley. This however doesnโt appear to have been followed by input from secondary mental health care. In 2021 there are a number of contacts with the crisis line and referrals to CMHT (working age), outlining concerns about low mood, anxiety and suicidal thoughts for the past 7 years since her mother passed away. Other contributory factors noted included the death of a neighbour the previous year and being charged with criminal damage and attending court in April 2021. Ms Myers had also spoken about the death of her brother and developing depression and an eating disorder as a result of discrimination at work. Another referral to CMHT in 2021 reportedly from Southwark council outlined concerns of Ms Myers feeling police were being sent to kill her. There was also a phone call from a mental health practitioner from the University of East London (student wellbeing service) expressing concerns about Ms Myersโ mental health and possible bizarre and paranoid behaviour.
In 2022 Ms Myers was assessed following a GP referral by the Southwark Assessment & Liaison Team. This assessment noted mental health difficulties appearing to commence in 2014 when Ms Myers was reportedly persecuted in her place of work, having worked as an early yearโs practitioner. It was reported that Ms Myers was being accused by her nursery in Southwark of being โuncooperative, rude and confrontationalโ, and was under investigation, and then left due to ill health. Following this, she received 12 sessions of Cognitive Behavioural Therapy (CBT) at the Maudsley. Following this, Ms Myers had reported not being able to gain paid employment, but had worked voluntarily in mental health research, been involved in various organisations and online podcasts, and was also featured in a book about Windrush. Ms Myers had reported an incident of the police in 2021 attending her home and beating her, and that she now has a criminal record and has been turned down for jobs due to her criminal record check (CRB). She had reported feeling she has been targeted by certain people she used to work with and there was a pattern. The impression of this review was that there was not any clear evidence of depression or psychosis, that past events have been very stressful and that Ms Myers could benefit from further talking therapy. She was then referred to IAPT.
In 2023 and going into 2024 there were a number of referrals to CMHT, due to concerns about Ms Myersโ mental health. There was reference of Ms Myersโ housing association seeking legal action due to antisocial behaviour. In December 2023 Ms Myers was reported to have called 111 herself reporting feeling hopeless about the threat of eviction from her home and having ongoing issues with her neighbours. In March 2023 Ms Myers was brought to A&E by the London Ambulance Service reporting worsening mental health. No evidence of thought disorder or psychosis observed and GP was advised to review Ms Myers and consider a referral to mental health services.
In September 2024 Ms Myers was referred to the Southwark Older Adult CMHT by Dennis Kamara, mental health nurse in primary care. There were concerns related to her behaviour and posting references to suicide in her social media. Ms Myers was seen by Dennis in primary care alongside her son Kevin who had travelled from Jamaica where he lives. There were reported issues with housing, neighbours and family. She has lived in her current address for 23 years and has been having issues with neighbours banging her door and making malicious complaints about her to the police. The housing association has reportedly taken an injunction against her and she is at risk of eviction and the case is currently in court. Her son had reported the reason for this being Ms Myers putting up inappropriate material online about the council and refusing to take it down. Her son had mentioned Ms Myers storing urine for months to throw it at her employerโs property and putting up comments on social media about feeling suicidal, which Ms Myers admitted to, however, stated she would never do this due to her Christian faith. There was also concern from Ms Myers about a camera being placed in the shared communal doorway by her neighbours, that it initially faced towards the street however now has been turned directly at her. She had reportedly denied feeling the neighbours watch her when she is in her flat however, the only issue is when she is coming through the communal front door.
Recent progress
Ms Myers was initially assessed by our team on 08/10/24 at home by Joseph Mukuba, Community Psychiatric Nurse. Following this, I decided to assess Ms Myers myself to help come to a psychiatric diagnosis and consider treatment options.
I have subsequently reviewed Ms Myers in clinic on 16/10/24. During this assessment Ms Myers described to me in detail her background history to date, including the trauma in her upbringing (physical abuse from mother, father dying when she was a young age), and her first marriage to a controlling husband resulting in domestic violence. Ms Myers talked about losing her mother 10 years ago, and around the same time being forced out of working as a nursery nurse and following this receiving therapy. Ms Myers talked about the issues with her neighbours persecuting her, which have worsened over the years, and her recent bereavement of her husband a few months ago.
On 23/10/24 Ms Myers contacted 111 and LAS due to feeling in โcrisisโ after her grandniece left having stayed with her without saying goodbye. I visited Ms Myers at home on the 25/10/24. During this visit, Ms Myers described in more detail the persecution she reports experiencing from her neighbours, whom she believes are trying to kill her, leading to her feeling unsafe at home. She reported since her husband passed away, she has been more careful in locking her front door and that if she didnโt do that โI would be deadโ. Ms Myers showed me the doorbell camera her neighbour set up on the outside of the shared front door, and how she believes the neighbour is using this to monitor her coming in and out of her home. Ms Myers talked about her neighbour kicking on her front door every day to provoke her and her belief that her neighbours want to try to provoke her into confronting them, which would then give them an excuse to harm and kill her. Ms Myers has explained to me how she believes other neighbours are also siding against her and that she also feels under threat from the police and housing association, all working together against her.

I summarised the purpose of review to complete the assessment following the referral to our team from Dennis Kamara, primary care mental health nurse.
I asked Ms Myers how she has been since we last met. She talked about the housing association not coming yet to fix her toilet seat, as well as the mould on her kitchen door and the damp in her windowsill. She reported attending her drumming group and doing a performance and spoke positively about this. She brought up about her neighbours continuing to try to provoke her since we last met. Ms Myers spoke about the neighbours โstompingโ on the floor loudly to provoke her. Ms Myers agreed that she believed the neighbours were trying to kill her. I recalled her previous statement to me that she believed the neighbours were trying to provoke her so that she would in turn confront them, and this would give them the excuse to try to kill her. Ms Myers accepted this is what she believed. She stated she would not attack them herself and is trying to keep to herself. Ms Myers said recently she stayed at a friendโs house overnight as she didnโt feel safe to return home. I asked about the camera. Ms Myers continued to report that she believed the camera on the doorbell was there to monitor her movements. She said she is trying not to think about this. She stated being more frustrated about her doorbell always being rung.
Ms Myers made a statement about never wanting to โgive upโ. I enquired about whether she experiences suicidal thoughts. Ms Myers stated this has been misconstrued and referenced a poster or piece of literature she was pictured in, that had the quote about people being unemployed, being at risk of suicide, and said this quote was then attached to her. Ms Myers did admit to having last had a suicidal thought in August this year, when her son was visiting her and she talked about her door being kicked. She didnโt report having experienced any plans or intent to act on this. Ms Myers stated there was no chance that she would come to harm via her own means. She has previously stated her Christian faith as a protective factor.
I summarised to Ms Myers what she had explained to me over the last 3 meetings leading to my opinion about her mental health diagnosis. This included Ms Myersโ description of her earlier life and trauma she experienced, the domestic violence in her first marriage which led to her separating and being saved by the housing situation, the loss of her mother 10 years ago around the same time as stopping working as a nursery nurse. Then over recent years with the increasingly difficult situation with her neighbours, beliefs that they are provoking her by monitoring her movements, trying to kill her and banging on her door, leading to her receiving a possession order which may potentially result in her being evicted and a possible custodial sentence due to being in contempt of court. Then more recently the stress of losing her husband. I explained to Ms Myers my impression that as a result of the build up of stress over these years that her beliefs and experiences about the neighbours could be explained by her being unwell with paranoia and psychosis. I explained that this is a condition that we could provide treatment for, including both antipsychotic medication, and psychosocial support, including psychology for psychosis. Ms Myers said in response to this that she had experienced some paranoia in the past. She said she accepted this was my explanation of opinion about her mental health. Ms Myers said she was willing to try medication if it would help. Ms Myers explained she had recently been to her new GP for vaccinations and blood tests, I advised I would check the results of these and then give her a call next week to see whether she would like to go ahead with starting antipsychotic medication. She agreed to this and accepted a patient information leaflet about risperidone. She confirmed having our contact numbers in case needing to get in touch in between appointments. Hawa confirmed she would make an appointment with Ms Myers when she had started on her medication to monitor how she is doing. We also discussed social activities and community groups, Ms Myers knew of Time & Talents and didnโt want to attend there, however appeared keener in Blackfriars.
Mental state examination
Ms Myers presented on time, she was well kempt, wearing a diabetes charity t-shirt, engaged well, reasonable rapport established, maintained eye contact, speech slightly pressured however, interruptible and voice not raised, mood reported as well and objectively appeared euthymic with reactive affect. Mild thought disorder evident with circumstantial responses. Describing persecutory delusional thoughts about neighbours trying to kill her and monitoring her movements. No reported auditory hallucinations and didnโt appear to be responding to unseen stimuli. Appeared well orientated to time and place. Insight partial however took on board opinion about diagnosis and agreed to consider treatment options explained.

I am writing as a competent woman, to formally address significant concerns regarding a report dated 01-11-2024, attributed to you or produced using your electronic signature. This report contains inaccuracies, omissions, and potential defamatory statements, all of which appear to have been made without lawful authority, and I do not consent.
Serious Breaches Identified
Inaccurate and Misleading Content: The report includes references to proceedings that allegedly affect me but omits critical details of my vulnerability as a grieving pensioner following the recent death of my lifelong companion. This oversight represents a failure of your duty of care, particularly when engaging with a vulnerable individual.

A copy of your qualification certificate, photo ID, and indemnity insurance certificate, confirming coverage of at least ยฃ5,000,000 for negligence, malpractice, or fraud.
Legal Implications of Non-Compliance
Failure to provide the requested documentation will be interpreted as implied admission of wrongdoing, and the contents of your report may be used as evidence in any subsequent legal proceedings. Be advised that falsifying information or failing to disclose required details may result in criminal liability under:

I met with Ms Myers in clinic at Marina House today alongside her allocated care coordinator, Hawa Oweh (CPN).
Background
Ms Myers is a 65-year-old female, not previously known to the Southwark Older Adult CMHT, however previously under working age mental health services. Ms Myers was referred to our team by Dennis Kamara, a mental health nurse in primary care. He requested for an assessment of diagnosis and treatment options, with information that Ms Myers has a long history of low mood and anxiety with some previous underlying trauma and recent stressors including her housing association taking her to court and ongoing disputes with her neighbours.
The following information about Ms Myersโs mental health background is taken from available electronic mental health records.
Ms Myersโ mental health records date back to 2017 where there is a Merlin report from police with concern about Ms Myers struggling with her mental health following a tribunal, with also a mention of her receiving previous support from the Maudsley. This however doesnโt appear to have been followed by input from secondary mental health care. In 2021 there are a number of contacts with the crisis line and referrals to CMHT (working age), outlining concerns about low mood, anxiety and suicidal thoughts for the past 7 years since her mother passed away. Other contributory factors noted included the death of a neighbour the previous year and being charged with criminal damage and attending court in April 2021. Ms Myers had also spoken about the death of her brother and developing depression and an eating disorder as a result of discrimination at work. Another referral to CMHT in 2021 reportedly from Southwark council outlined concerns of Ms Myers feeling police were being sent to kill her. There was also a phone call from a mental health practitioner from the University of East London (student wellbeing service) expressing concerns about Ms Myersโ mental health and possible bizarre and paranoid behaviour.
In 2022 Ms Myers was assessed following a GP referral by the Southwark Assessment & Liaison Team. This assessment noted mental health difficulties appearing to commence in 2014 when Ms Myers was reportedly persecuted in her place of work, having worked as an early yearโs practitioner. It was reported that Ms Myers was being accused by her nursery in Southwark of being โuncooperative, rude and confrontationalโ, and was under investigation, and then left due to ill health. Following this, she received 12 sessions of Cognitive Behavioural Therapy (CBT) at the Maudsley. Following this, Ms Myers had reported not being able to gain paid employment, but had worked voluntarily in mental health research, been involved in various organisations and online podcasts, and was also featured in a book about Windrush. Ms Myers had reported an incident of the police in 2021 attending her home and beating her, and that she now has a criminal record and has been turned down for jobs due to her criminal record check (CRB). She had reported feeling she has been targeted by certain people she used to work with and there was a pattern. The impression of this review was that there was not any clear evidence of depression or psychosis, that past events have been very stressful and that Ms Myers could benefit from further talking therapy. She was then referred to IAPT.
In 2023 and going into 2024 there were a number of referrals to CMHT, due to concerns about Ms Myersโ mental health. There was reference of Ms Myersโ housing association seeking legal action due to antisocial behaviour. In December 2023 Ms Myers was reported to have called 111 herself reporting feeling hopeless about the threat of eviction from her home and having ongoing issues with her neighbours. In March 2023 Ms Myers was brought to A&E by the London Ambulance Service reporting worsening mental health. No evidence of thought disorder or psychosis observed and GP was advised to review Ms Myers and consider a referral to mental health services.
In September 2024 Ms Myers was referred to the Southwark Older Adult CMHT by Dennis Kamara, mental health nurse in primary care. There were concerns related to her behaviour and posting references to suicide in her social media. Ms Myers was seen by Dennis in primary care alongside her son Kevin who had travelled from Jamaica where he lives. There were reported issues with housing, neighbours and family. She has lived in her current address for 23 years and has been having issues with neighbours banging her door and making malicious complaints about her to the police. The housing association has reportedly taken an injunction against her and she is at risk of eviction and the case is currently in court. Her son had reported the reason for this being Ms Myers putting up inappropriate material online about the council and refusing to take it down. Her son had mentioned Ms Myers storing urine for months to throw it at her employerโs property and
putting up comments on social media about feeling suicidal, which Ms Myers admitted to, however, stated she would never do this due to her Christian faith. There was also concern from Ms Myers about a camera being placed in the shared communal doorway by her neighbours, that it initially faced towards the street however now has been turned directly at her. She had reportedly denied feeling the neighbours watch her when she is in her flat however, the only issue is when she is coming through the communal front door.
Recent progress
Ms Myers was initially assessed by our team on 08/10/24 at home by Joseph Mukuba, Community Psychiatric Nurse. Following this, I decided to assess Ms Myers myself to help come to a psychiatric diagnosis and consider treatment options.
I have subsequently reviewed Ms Myers in clinic on 16/10/24. During this assessment Ms Myers described to me in detail her background history to date, including the trauma in her upbringing (physical abuse from mother, father dying when she was a young age), and her first marriage to a controlling husband resulting in domestic violence. Ms Myers talked about losing her mother 10 years ago, and around the same time being forced out of working as a nursery nurse and following this receiving therapy. Ms Myers talked about the issues with her neighbours persecuting her, which have worsened over the years, and her recent bereavement of her husband a few months ago.
On 23/10/24 Ms Myers contacted 111 and LAS due to feeling in โcrisisโ after her grandniece left having stayed with her without saying goodbye. I visited Ms Myers at home on the 25/10/24. During this visit, Ms Myers described in more detail the persecution she reports experiencing from her neighbours, whom she believes are trying to kill her, leading to her feeling unsafe at home. She reported since her husband passed away, she has been more careful in locking her front door and that if she didnโt do that โI would be deadโ. Ms Myers showed me the doorbell camera her neighbour set up on the outside of the shared front door, and how she believes the neighbour is using this to monitor her coming in and out of her home. Ms Myers talked about her neighbour kicking on her front door every day to provoke her and her belief that her neighbours want to try to provoke her into confronting them, which would then give them an excuse to harm and kill her. Ms Myers has explained to me how she believes other neighbours are also siding against her and that she also feels under threat from the police and housing association, all working together against her.

On review
I summarised the purpose of review to complete the assessment following the referral to our team from Dennis Kamara, primary care mental health nurse.
I asked Ms Myers how she has been since we last met. She talked about the housing association not coming yet to fix her toilet seat, as well as the mould on her kitchen door and the damp in her windowsill. She reported attending her drumming group and doing a performance and spoke positively about this. She brought up about her neighbours continuing to try to provoke her since we last met. Ms Myers spoke about the neighbours โstompingโ on the floor loudly to provoke her. Ms Myers agreed that she believed the neighbours were trying to kill her. I recalled her previous statement to me that she believed the neighbours were trying to provoke her so that she would in turn confront them, and this would give them the excuse to try to kill her. Ms Myers accepted this is what she believed. She stated she would not attack them herself and is trying to keep to herself. Ms Myers
said recently she stayed at a friendโs house overnight as she didnโt feel safe to return home. I asked about the camera. Ms Myers continued to report that she believed the camera on the doorbell was there to monitor her movements. She said she is trying not to think about this. She stated being more frustrated about her doorbell always being rung.
Ms Myers made a statement about never wanting to โgive upโ. I enquired about whether she experiences suicidal thoughts. Ms Myers stated this has been misconstrued and referenced a poster or piece of literature she was pictured in, that had the quote about people being unemployed, being at risk of suicide, and said this quote was then attached to her. Ms Myers did admit to having last had a suicidal thought in August this year, when her son was visiting her and she talked about her door being kicked. She didnโt report having experienced any plans or intent to act on this. Ms Myers stated there was no chance that she would come to harm via her own means. She has previously stated her Christian faith as a protective factor.
I summarised to Ms Myers what she had explained to me over the last 3 meetings leading to my opinion about her mental health diagnosis. This included Ms Myersโ description of her earlier life and trauma she experienced, the domestic violence in her first marriage which led to her separating and being saved by the housing situation, the loss of her mother 10 years ago around the same time as stopping working as a nursery nurse. Then over recent years with the increasingly difficult situation with her neighbours, beliefs that they are provoking her by monitoring her movements, trying to kill her and banging on her door, leading to her receiving a possession order which may potentially result in her being evicted and a possible custodial sentence due to being in contempt of court. Then more recently the stress of losing her husband. I explained to Ms Myers my impression that as a result of the build up of stress over these years that her beliefs and experiences about the neighbours could be explained by her being unwell with paranoia and psychosis. I explained that this is a condition that we could provide treatment for, including both antipsychotic medication, and psychosocial support, including psychology for psychosis. Ms Myers said in response to this that she had experienced some paranoia in the past. She said she accepted this was my explanation of opinion about her mental health. Ms Myers said she was willing to try medication if it would help. Ms Myers explained she had recently been to her new GP for vaccinations and blood tests, I advised I would check the results of these and then give her a call next week to see whether she would like to go ahead with starting antipsychotic medication. She agreed to this and accepted a patient information leaflet about risperidone. She confirmed having our contact numbers in case needing to get in touch in between appointments. Hawa confirmed she would make an appointment with Ms Myers when she had started on her medication to monitor how she is doing. We also discussed social activities and community groups, Ms Myers knew of Time & Talents and didnโt want to attend there, however appeared keener in Blackfriars.
Mental state examination
Ms Myers presented on time, she was well kempt, wearing a diabetes charity t-shirt, engaged well, reasonable rapport established, maintained eye contact, speech slightly pressured however, interruptible and voice not raised, mood reported as well and objectively appeared euthymic with reactive affect. Mild thought disorder evident with circumstantial responses. Describing persecutory delusional thoughts about neighbours trying to kill her and monitoring her movements. No reported auditory hallucinations and didnโt appear to be responding to unseen stimuli. Appeared well orientated to time and place. Insight partial however took on board opinion about diagnosis and agreed to consider treatment options explained.
Care Plan

Arrived in England on June 4th, 2024, accompanied by my brother. We should have arrived in May to attend the funeral service for Arnold Tomlinson, deceased husband of our mother.
We stayed with our mother for the duration of our visit. My brother spent 2 weeks, due to work obligations and I was able to spend 3 months.
ยท During our visit, we made aware of of events that were certainly having a negative impact on our mother. We were made aware;
ยท court case between our mother and HAW โ Housing Authority for Women.
ยท Conflict between our mother and her neighbors.
ยท Conflict with the police and our mother.
ยท Conflicts involving other persons and our mother.
The court case involved the possibilities of eviction and imprisonment. We were fortunate to attend the court case and got some positive news that there was a change of heart; the imprisonment and eviction were put on hold.
We witnessed the arguments between our mother and the neighbors a couple of times. During my duration, the police arrived at least 2 times. I was interviewed by the police on 2 occasions.
We had a few arguments and disagreements of our own due to our differences in opinions on a few matters, to include the behavior and mindset of our mother.
I must add that on my leaving England, in my opinion, my mom a was in a better frame of mind than when I arrived.
Just to add that, we encountered some issues with the apartment that resulted in discomfort. There was a broken toilet that was addressed. A leak in the bathroom that was also rectified. The water in the kitchen sink was draining extremely slow and this affected us severely. This situation lasted in excess of a month.
I have no intention for you to TRIGGER ME… I responded to your email under DURESS using the REASONABLE ADJUSTMENTS, so as not to miss your 7 days deadline in case I get the Atypical Parkinsonism and unable to WRITE and function to my normal schedule. I will, therefore, not be getting into anymore dialogue than is absolutely necessary. I CORRECTED THE inaquracies and expecting you to get on with your work. Today is the date I lost my HUSBAND, and I would appreciate if you gave me the chance to GRIEVE for him and my MOTHER who I lost 11 years ago and has not been given the time to grieve.
I am sure there are CONCERNS that you are refusing to see if you take the time over the 7 days you gave me to respond.
I did not ask you to address anything that does not fall within the remit of the MPS. I was just correcting the OBVIOUS mistakes you made.ย
As long as you act in a professional MANNER in investigating the CRIME, I do not expect you to investigate the COMPLAINT based on your UNDERSTANDING of my latest concerns, but on the FACTS that are presented and can be VERIFIED.

Media volunteer newsletter – April 2025 Dear Media Volunteer,April is bowel cancer awareness month and as such, a particularly busy time of year for MVL.
Between the Bowelbabe Fund, Bobby Moore Fund and more recent focus on bowel cancer created by the likes of the inspirational Dame Deborah James, it’s an area where we increasingly rely on media volunteers with experiences of bowel cancer to share their stories. Media volunteers like Ellie (pictured with her fiancรฉ Rory and dog Alfie), who was diagnosed with stage 4 bowel cancer but, happily, is now living cancer free.
The 28-year-old from Peterborough has spoken to the media for us, recently been filmed as part of our work with our Partnerships team and, just last week, appeared in front of hundreds of Cancer Research UK staff as part of a panel at an internal meeting discussing progress around bowel cancer treatment.
Away from bowel cancer, we’ve also asked media volunteers to share their stories for a blog about radiotherapy treatment and also had young people with a cancer experience included in an article for our Digital News webpages. I can appreciate that this is very difficult for you. I also undertook further research on your behalf in Southwark and sent it to you. i am sorry to learn that you are so overwhelmed but it will be normal for anyone in your circumstances. please consider going in person and requesting help, a support worker from Southwark Council will be assigned to you and the Mayor and 2 other Councillors were emailed yesterday. i would encourage you to find a church near you and do your best to try to help yourself as much as you can. i have spent at least another hour today drafting an email to Anthony updating him following the court hearing and responding to you. I can appreciate it is very difficult for you but although I have sympathy and compassion for you I cannot give you much more of my time and attention . Please be advised that I met the above cited individual last week when I attended at Court with her for an eviction hearing at Clerkenwell and Shoreditch County Court on 20th March 2025 brought by Southwark Housing for Women, followed by a committal process.
In the second case of a committal hearing the Judge awarded a two year suspended custodial sentence of Mrs Mervelee Myers.
Please be advised that as an advocate in the community with significant experience of representing vulnerable individuals and communities I have experience representing the vulnerable who find themselves, homeless, dispossessed, not in receipt of benefits and advocating on their behalf mostly to a raft of disparate Councils as well as representations to the DWP for benefits / reinstatement of benefits and representations for access to health services and referrals.
Please be advised that I have volunteered for decades and also volunteer directly with refugees and indirectly (via Haringey Council historically) and am also recognised as a Family Mosaic Community Champion. I have also volunteered for Victim Support to give emotional and practical support for victims and witnesses of crime.
Please be advised that I met Mrs Myers and took a brief history from her last Thursday 20th March 2025 where she advised that she has no current GP and is unable to elicit the NHS for assistance, despite significant health problems, including but not limited to blood pressure problems, Parkinson’s and anxiety and panic attacks.
She also advised me that she has suffered discrimination and harassment and multiple safeguarding failings whilst resident at the above property.
Having decades of experience as a volunteer I am concerned for the well-being of Mrs Myers who I believe is suffering from historic and recent trauma and may suffer from cognitive impairment and is very overwhelmed. Mrs Myers advises me that she does not have a support worker If you could kindly expedite the relevant referral urgently to Adult Social Services I would be obliged wherein Mrs Myers may be able to expand on her challenges and limited circumstances.
I would be most obliged if you would be kindly enough to arrange a Needs Assessment urgently for this vulnerable individual at your earliest convenience and involve the appropriate Councillors for support including but not limited to Councillor Evelyn Akoto, Cabinet Member for Health and Well being and also Councillor Sam Dalton, Deputy Cabinet Member for Supported Housing.

Dear Zaiba Quershi acting as CEO,
Upon accessing the customer portal today, I observed with serious concern that all data relating to my account has been inexplicably removed, despite your legal obligation under the Limitation Act 1980 and Data Protection Act 2018 to retain such records for a minimum of six years.
I trust this is an error and expect the full data set to be restored promptly from your system backups, in accordance with your statutory data retention obligations. Once restored, the records will confirm that there have been no valid court proceedings issued โ specifically no Injunction, no ASBO, no Claim for Possession, and no valid Notice filed with the Local Authority or issued by any competent court. This absence of legitimate court documentation is deeply telling.
You are already aware that I submitted a lawful and documented request via the official RESOLVER system (Ref: 10993593) on 28-02-2025, which you chose to ignore. Your continued silence and failure to produce any valid court-issued documents strongly indicate that you were aware no such filings existed โ a matter now forming the basis of a criminal complaint and a Statutory Declaration, both of which are attached to this notice for the record.
Serious Criminal Allegations
The conduct described above appears to constitute a coordinated extortion racket, involving:
Knowingly unlawful data access and use in violation of the UK General Data Protection Regulation (UK GDPR)
Threats and intimidation towards a vulnerable individual without legitimate legal authority
Conspiracy to Defraud (see R v Hollinshead [1985] AC 975)
Mail Fraud through misrepresentation and deceptive correspondence
Forgery and Counterfeiting of legal documents contrary to the Forgery and Counterfeiting Act 1981
Trespass and unauthorised surveillance
Misuse of private information (Campbell v MGN Ltd [2004] UKHL 22)
Child endangerment and abuse, through unlawful pressure impacting dependents
Defamation under the Defamation Act 2013
Conspiracy to Pervert the Course of Justice
Violations under the Proceeds of Crime Act 2002 (POCA)
Moreover, your actions are causing alarm, distress and public humiliation, especially to vulnerable persons โ which can also engage Section 2A of the Protection from Harassment Act 1997, and may be deemed gross misconduct under the Health and Safety at Work etc. Act 1974, where employers owe a non-delegable duty to protect individuals from psychological harm.
Potential Criminal Penalties
These are not mere procedural failings. They are potentially indictable criminal offences, carrying substantial prison sentences, asset forfeiture, and permanent reputational damage. If prosecuted:
Conspiracy to Defraud carries up to 10 years imprisonment
Data Fraud and Misuse under DPA 2018/UK GDPR โ fines up to ยฃ17.5 million or 4% global turnover
Forgery or Counterfeit Legal Filings โ up to 10 years imprisonment
Harassment or Intimidation โ 6 months to 5 years, depending on severity

You have been advised by MCP & H4W that this would fall under tenant responsibility, which you disagree with as the toilet seat was broken shortly after the seat was fitted, thus you reported this to MCP as a faulty/defective toilet seat. You also contacted H4W Customer Services Team to advise of the issue as you were unable to get hold of MCP. You informed Customer Services of the situation and advised that the top part of the toilet has broken off from the basin of the toilet which was due to no fault of your own and that it required repairing/replacing. Customer Services reiterated the circumstance to Repairs who advised that this would still be the tenantโs responsibility which you are dissatisfied with and are still requesting for the toilet seat issue to be resolved. You sent a follow up email on 24/09/24 after receiving your stage 1 acknowledgement outlining for MCP specifically to be included and requesting compensation for your friendโs phone falling down the toilet as a result of the toilet seat being broken. I understand that to resolve your complaint you are seeking a repair of your toilet seat and compensation for a friendโs phone that fell down the toilet as an apparent result of your toilet seat being broken. He also wrote down instructions on how to operate it. I therefore deem this part of your complaint not upheld. I reviewed our contractor, MCPโs portal to investigate the repairs reported regarding your front door. There are no reports in the past year, of a repair being raised regarding a fault to the lock on the front door. I therefore deem this part of the complaint not upheld. Oninvestigating the communication between H4W and yourself, I was unable to find evidence that demonstrated we had been clear with a decision regarding the tree removal. I can confirm that it is the residentโs responsibility to maintain the garden. This will include the removal of a tree. Wearesorry we were not clear in our communication and for the delay in responding back to you. This section of the complaint is upheld. It was reported on the 23rd February 2022 a repair to the gate and fence was required. A make safe was carried out but inline with our policy, it is the residentโs responsibility to repair/replace a fence that divides two gardens. After reviewing the photos held on the contractorโs portal that the fence in question divides two gardens. We therefore are unable to fulfil your request by repairing/replacing the fence. Onthe 22nd May 2022 it was reported to our repairs contractor, the communal door had a smashed pane of glass. A board up was completed to the broken glass on the 22/05/2022. The glass was replaced on the 18th July 2022 which is outside of our standard timescales. I therefore deem this section of the complaint upheld. I have requested an anti-social behaviour case is opened on your behalf. My colleague from the complaints department will contact you with the case reference number. I was unable to find any evidence of a complaint being logged relating to the above issues you have raised with us. Therefore, I am unable to review the complaint handling. I amsorry you had to report a staff member terminating your call when you contacted us. We take reports of poor conduct by our inhouse staff and contractors very seriously. We appreciate that you found this experience unpleasant, and we assure you that it was not representative of the services we aim to deliver. This concludes Stage 1 of our internal complaints process.
