My name is Mervelee Myers who was made an INACTIVE PATIENT on the 6th December 2024. I was asked by the Dentist treating me to get my Blood Test results as part of the treatment. I would appreciate if I could get same ASAP for my next appointment. The BT result is needed to know what treatment is best to save me losing all my teeth as a result of having diabetes and other underlying conditions.

I was not asking to do a Blood Test, I am asking for the last Blood Test results when I was booked to have a Review on the 5th December 2024 and got to the appointment and told I am INACTIVE PATIENT. I have some recordings.
Please let me reiterate that I need the last test for the Dentist ASAP. I was told over the phone I need to come in and do Subject Access Request.
I don’t want to come in and be accused of anything, so before I come in I would appreciate if some guidelines are sent to me.
I was told someone would contact me about the reasons I was made an INACTIVE PATIENT. A month later I who is a DISABLED NHS PENSIONER is without a GP.
I would appreciate if you can let me know when to come in to get my DATA for the Dentist.

Yes, as you mentioned you are an inactive patient and not registered with us. Unfortunately we cannot book you for a blood test due to this. If you have changed practice, you can contact them for a blood test request. You can re-register with us by collecting a form at reception or via online, please be advised it can take up to 48 hours for this to be added to the system.
