The form was not attached before, I got an attachment to YouTube. I have since had the form from DSA and will take it to the GP to get sorted. It would have been easier if the terms were explained and clarified. I am keeping records as am tired of what’s happening to me. Accused of making up disabilities https://www.gov.uk/employment-tribunal-decisions-ms-m-myers-v-london-early-years-foundation-2300047-2016 and the latest happenings do nothing for my anxiety.
Thanks.
Mervelee.

Hi Mervelee
Hope you are well. I have reviewed your medical evidence. Unfortunately, we cannot accept medical record print outs, and in additional the evidence is quite old. I have attached a medical evidence form in this email. Please can you request your GP completes it . Please can you then send us a copy/pictures of each page of the form please?
DSA will most likely also require a copy, so I recommend uploading it to the SFE account too or emailing it to dsa_team@slc.co.uk.
Warm Regards
Martin
